Step 1: Complete Registration Form
All families should complete this fillable form to register your child in one of Rainbow Station's programs.
Step 2: Tell Us About Your Child
If You Have a Preschool Child:
Families enrolling your child for the Nursery School, Preschool, Private Kindergarten, or Get Well Place programs should complete this fillable information form so we may begin to get to know your child.If You Have a School-Age Child:
Families enrolling your child for The Village program should complete this fillable information form so we may begin to get to know your child.
Step 3: Complete a Health and Immunization Form
Families enrolling your child for Nursery School, Preschool, Private Kindergarten, or Get Well Place programs should complete one of the information forms below so we can ensure your child is up to date on medical requirements for school.
Step 4: Submit Your Information to the School of Your Choice
Download and fill out the above forms and then email (or fax) the completed form to the location of your choice. Click here to find your location.
