In the spirit of partnership, and to make it as easy as possible for you to provide us with the information we need to provide excellent care for your child, we provide a variety of key documents for you to access at your convenience—anytime, anywhere. Please let us know if you have any questions about how to complete any of these documents, about how a specific document is used, or if we can help you by providing additional documents for you and your family to use at our school.
Forms for Enrollment
Registration Form: All families should complete this form to register your child in one of Rainbow Station's programs.
Getting to Know You: Families enrolling your child for the Nursery School, Preschool, Private Kindergarten, or Get Well Place programs should complete this information form so we may begin to get to know your child.
The Village Getting to Know You: Families enrolling your child for The Village program should complete this information form so we may begin to get to know your child.
Health and Immunizations: Families enrolling your child for Nursery School, Preschool, Private Kindergarten, or Get Well Place programs should complete this information form so we can ensure your child is up to date on medical requirements for school.
Other Forms
Authorization for Alternate Pick Up: Please use this form to inform us of additional individuals who are authorized to pick up your child. This form MUST be signed and presented in person. Please note: All additional authorized individuals must present identification prior to releasing your child.
Change of Information: Please use this form to notify us of ANY changes in personal information for you or your child.
Provider Certification Form. Please use this form to verify your child's enrollment to qualify for your employer's Dependent Care Assistance reimbursement.Parent Exit Interview:
